I hereby give permission for my child/children to take part in the 2025 FMDQ-Next Summer Camp Programme (the “Programme”). I understand that visual media will be taken during the Programme and may be used to keep journal of activities, shared during Programme presentations and/or included in reports for journalistic or promotional purposes including flyers, brochures, newspapers and on the internet, which will be available locally and internationally. I understand that although my child/children’s visual media may be used for the aforementioned purposes, his or her identity will not be disclosed without prior consent. I do not expect compensation and agree that all photos are the property of FMDQ Group PLC (“FMDQ Group”) and its affiliates
I hereby covenant with FMDQ Group to pick my child/children up by 2:00 PM, if admitted into the Programme
I will not hold FMDQ Group accountable for handing over my child/children to my designated person for pickup during the Programme
I hereby give my approval for my child/children’s participation in any and all activities prepared by FMDQ Group during the Programme. In exchange for the acceptance of said child/children’s candidacy by FMDQ Group, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless FMDQ Group and all its respective officers, agents, and representatives from any and all liability for injuries to said child/children arising out of travelling to, participating in, or returning from selected Programme sessions
In case of injury to said child/children, I hereby waive all claims against FMDQ Group
In case of a medical emergency, I give permission to FMDQ Group to take reasonable steps to care for my child/children, including taking them to an FMDQ Group designated hospital
I confirm that I am the parent or legal guardian of the child/children registered for this event. Where child/children is used above, this also refers to ward/wards
